To start in reverse: A Duvet - Quilted blanket type bed cover, also known as a Continental Quilt, which sometimes contains Duck down, but now mostly contains fibers, the mass of which is stated in 'Tog' values. It is usually larger than the bed in size and extends in folds over each side and end of the bed. Bed sheets - The lower sheet covers the mattress whilst the upper one is used between the duvet or blankets and the lower sheet. Usually made of cotton, but could be nylon or similar. If classed as 'fitted sheet' the lower one has elasticated edges which slip over the mattress. Bed spread and bed cover are basically the same. Used for tidiness when the bed is not in use. Again, generally speaking no other bed material is placed on top of the duvet - the idea being to keep it 'puffed up'. A spread is normally a thin blanket over the top for looks. I believe a duvet is fabric from the mattress to the floor to cover dust and monsters hiding under the bed.
